LEGACY 716 LLC ACTIVITY / SPORTS CAMP WAIVER & CONSENT TO TREAT AND RELEASE
I hereby give permission to the officers, employees, agents, trainers, coaches, and staff of Legacy 716 LLC for all camps I have registered for. To take whatever action is necessary for the health and welfare of my child including consenting on my behalf to any and all medical treatments, procedures, operations and/ or hospitalizations. I understand that sports camps involve physical activity that can result in serious injury. I further agree to hold Legacy 716 LLC and the aforementioned staff members harmless, indemnify them for all medical bills incurred for the treatment of my child, and release them from liability for any injuries incurred by my son while attending the camp.
Signing this form verifies that I have been informed of the risks and possibility of injury involved with participating in physical activities as part of Legacy 716 LLC programming. I understand that an infraction of the rules or regulations may result in an injury to self or others. I also understand that no modification of protective equipment or uniform should be made and that all equipment is expected to be used in the manner for which it is intended. I understand that it’s my responsibility to report faulty or ill-fitting personal equipment to a staff member immediately. I also agree to not use any Legacy 716 LLC equipment or facilities without proper supervision.
It’s my responsibility to report all injuries and disclose all medical issues that may hinder the ability to participate in physical activities or athletics. I’m responsible for the follow-up care and treatment of any injuries incurred and assume all risks, responsibilities and possible consequences associated with participating in physical activities or athletics. It’s my responsibility to update medical information in a timely manner and inform Legacy 716 LLC of any restrictions or risks. Legacy 716 LLC reserves the right to deny access and / or participation at any time to participants who display any medical / injury related restriction.
